It exists to protect you — it confirms the plan agrees the treatment is necessary, checks your tooth against the plan’s guidelines, and sets out exactly what the insurance will pay, so no unexpected bill arrives later.

Routine cleanings, regular exams, cavity-check X-rays, and emergency visits for pain relief never require prior authorization. Major treatment almost always does:

  • New crowns
  • Foundation core buildups
  • Back tooth (molar) root canals
  • Deep gum cleanings
  • Full or partial dentures
  • Denture relines
  • Surgical extractions
  • Jawbone smoothing (alveoloplasty)

Dual-Eligible Plans (D-SNP) in Maryland

If you qualify for both Medicare and Maryland Medicaid (administered through Maryland HealthChoice and the Maryland Department of Health), you can enroll in a Dual Eligible Special Needs Plan — for example Johns Hopkins Advantage MD D-SNP, UnitedHealthcare Dual Complete, or CareFirst Dual Advantage.

Maryland’s Medicaid program provides comprehensive adult dental benefits, which makes dual-eligible coordination unusually clean. Medicare is your primary coverage and Maryland Medicaid acts as the secondary payer, so covered dental procedures carry $0 copayments, $0 deductibles, and full coverage up to program limits. Under federal and state balance-billing rules, our office may not bill dual-eligible patients for remaining balances on covered procedures.

Many Maryland D-SNP plans also include a supplemental flex spending card with an annual allowance between $1,500 and $3,500 that can be used at the dental office toward dentures, crowns, deep cleanings, and other essential care.

Plan Guidelines, Annual Limits & Exclusions

A few practical rules decide how far your Medicare dental benefit goes:

  • Annual dollar maximum: most standard Medicare Advantage plans cap the benefit between $1,000 and $2,500 per calendar year, while enhanced D-SNP plans may offer $3,000 or higher. Care beyond the cap is your responsibility at discounted network rates.
  • Deductibles and coinsurance: cleanings and checkups have no deductible. Basic fillings and major crowns may carry a small annual deductible of $50 to $100, with coinsurance typically 20% to 50%.
  • Replacement waiting clocks: crowns and complete dentures are usually covered once every 5 years (60 rolling months). A partial or full denture fitted three years ago will not be replaced until the full five years have passed.
  • Missing tooth rule: some Medicare Advantage contracts will not cover replacing a tooth with a bridge or partial denture if that natural tooth was extracted before your enrollment date with the insurer.
  • Common exclusions: routine Medicare Advantage dental riders do not cover purely cosmetic treatment such as teeth whitening, cosmetic porcelain veneers, or sports mouthguards. Dental implant bodies and implant-supported prosthetics are excluded unless your plan includes an upgraded implant rider.

Under balance-billing rules we cannot bill you for the remainder on covered procedures.

What is never covered?
